Vial of Life
What is the Vial of Life?
The Vial of Life is a program that is designed to provide important Information about you when you are unable to speak for yourself.
What does it Cost?
It is FREE for all participants.
How does the program work?
The program works by filling out a simple questionnaire about your medical history. When the questionnaire is completed it should be placed on your refrigerator. Participants will be given a Vial of Life decal that should be placed on their door to alert Emergency Responders. The questionnaire will provide emergency responders with the following information:
- Patients name
- Age
- Drug allergies
- Primary Care Physician
- Current medications
- Past medical history
Is there a need?
There are several different reasons why there is a need for the Vial of Life program. Often, responders go to emergencies where the patient is unable to speak or may be confused. The Vial of Life program will speak for these patients when they are unable to.
We encourage you to utilize this program so that we may provide the best customer service possible.
