Appearance/Court Date Reset Request*

This form must be filled out, received and GRANTED by the Court BEFORE or ON the appearance or court date. It is NOT sufficient for the Court to simply receive the request. It must be GRANTED. Upon receipt and determination that the Defendant has NOT received a prior reset on this case, the Defendant will be set on the next available docket to appear before the Judge, and notification of the new date will be sent through the mail. Otherwise, the Defendant MUST be present at the date and time originally scheduled. The Judge currently conducts sessions on the second (2ND) Wednesday of every month. Only one (1) reset will be granted, and all extensions are FINAL. Resets CANNOT be made by phone.

This form must be filled out and received by the Court ON or BEFORE the required date. Full payment must accompany this form.

Driver Record Request (Online)

This request must be completed online, and the Driver Record returned to the Court ON or BEFORE the due date. If this is needed as part of the completion requirement for the Driving Safety Course, the Defendant must login to the application in order for eligibility to be determined and submit $12.00. The Driver Record will be mailed by the Texas Department of Public Safety within five (5) BUSINESS days.

Driving Safety Course (DSC) Application & Affidavit Form*

This form must be filled out, notarized and received by the Court ON or BEFORE the appearance date. Payment, copy of a valid Texas driver's license or permit, and a copy of a current liability insurance card must accompany this form.

Incarcerated Time Served Request*

This form allows a Defendant who is currently in jail or has been released from jail to formally request credit for time served. This is NOT the appropriate venue to request that a Defendant be able to lay out a fine prior to going to jail. This request must be made in person at the Court and transport to the Denton County Jail arranged.

Plea Form*

This form enables the Defendant to enter a plea to the Court. Should the Defendant wish to pay the fine, and thus waive his right to a jury trial, a plea of GUILTY or NOLO CONTENDERE (NO CONTEST) must be entered. If the Defendant chooses to submit the fine by mail and does not indicate a plea, NO CONTEST will be assumed. In the event the Defendant requests either a Driver Safety Course or Deferred Disposition, a plea of GUILTY or NO CONTEST must be entered, and the right to a jury trial waived. For all NOT GUILTY pleas, the Defendant will be placed on the next available docket to speak with the Judge, and a PRETRIAL HEARING will be scheduled at that time. NO ONE other than the Defendant can enter a plea.
